What is Peripheral Stimulation?

Peripheral Nerve Stimulation (PNS) is a neuromodulation therapy designed to help manage chronic, localized pain that has not responded adequately to traditional treatments. This minimally invasive procedure involves placing a small electrical device near a targeted peripheral nerve to deliver gentle electrical impulses that help regulate pain signals.

The peripheral nervous system consists of all the nerves located outside the brain and spinal cord. When these nerves become injured, irritated, or are malfunctioning, they may transmit persistent or abnormal pain signals to the brain, contributing to ongoing discomfort. PNS works by modifying these signals to “trick the brain” to help reduce pain and improve function.

Peripheral Nerve Simulation (PNS)

The Benefits of PNS

Most cases begin with non-surgical treatments aimed at reducing inflammation and strain on the plantar fascia.
Targeted Pain Relief

It directly targets the specific nerve causing the pain, often providing more precise relief than broad, systemic (body-wide) treatments.

Non-Opioid

It is a drug-free option that can reduce or even eliminate the need for oral pain medications, including opioids.

Minimally Invasive

The procedure is less invasive than traditional surgery, which means a faster recovery and lower risk of complications.

Reversible

If the treatment is not effective, the device can be turned off and removed. Additionally, patients still have options for more invasive surgical therapy if peripheral nerve stimulation doesn’t provide suitable relief.

What Conditions are Treated with PNS?

PNS can be used to treat a variety of chronic pain conditions, including:

  • Neuropathic pain: Pain caused by nerve damage.
  • Complex Regional Pain Syndrome (CRPS): Especially when it is localized to a specific limb or area.
  • Chronic headaches: Such as occipital neuralgia or certain types of migraines.
  • Post-amputation pain: Including phantom limb pain.
  • Post-surgical nerve pain: Pain that persists after a surgical procedure.
  • Chronic joint pain: Such as chronic knee or shoulder pain.
  • Post Arthritic pain: Such as chronic back, neck, shoulder, arm, knee pain.

Is PNS right for you?

PNS is a targeted therapy, so it is most effective for pain that is localized and can be traced to a specific peripheral nerve. It is often recommended for patients who have:

  • Chronic pain that has lasted for several months or more.
  • Failed to find adequate relief from other treatments like physical therapy, medication or nerve blocks.
  • Pain in a specific, well-defined area.

Our Approach at IMIIT

Our process for implanting a PNS device involves two phases:

1. Trial Period:

A temporary electrode is placed next to the nerve and connected to an external battery that the patient wears. This allows the patient to “test drive” the therapy for a few days to see if it provides a significant reduction in their pain. If the trial is successful (usually defined as at least a 50% reduction in pain), the patient can proceed to the permanent implant. After one week of clinical trial, all leads are removed to prevent any complications.

2. Permanent Implantation:

The permanent device is implanted in a minimally invasive, outpatient procedure. The leads are placed and secured, and a small pulse generator is placed under the skin. After placement, the patient recovers at home, no hospital stay is required.

Recovery is usually quick, with most patients able to return to light activity within a day or two and full activity within a week or two.
