Overview:

Plantar fasciitis is one of the most common causes of chronic heel pain. It occurs when the thick band of tissue on the bottom of the foot—called the plantar fascia—becomes inflamed or irritated. While many people improve with conservative care, persistent plantar fasciitis can significantly limit daily activities, mobility, and quality of life. At IMIIT, our interventional specialists offer advanced, minimally invasive treatments for patients who have not found relief through stretching, physical therapy, orthotics, or injections. These procedures target the source of pain to help you move comfortably again—without surgery and with minimal downtime.

Initial & Conservative Care

Most cases begin with non-surgical treatments aimed at reducing inflammation and strain on the plantar fascia.
Rest & activity modification:

Reducing high-impact activities and incorporating low-impact exercise

Stretching:

Targeting the plantar fascia, Achilles tendon, and calf muscles—especially before getting out of bed

Ice therapy:

Applying ice or rolling a frozen water bottle under the foot

Supportive footwear & orthotics:

Shoes with proper arch support and cushioning.

Night splints:

Keeping the plantar fascia stretched overnight to improve morning pain

Over-the-counter medications:

Such as ibuprofen or naproxen

Physical therapy:

Including stretching, strengthening, and taping techniques.

When these measures fail to resolve pain, advanced minimally invasive treatments may be beneficial.

Minimally Invasive Treatment For Plantar Fasciitis at IMIIT

Our team offers two highly effective, image-guided treatments for chronic plantar fasciitis:

Plantar Fasciitis Embolization (PFE)

Plantar Fasciitis Embolization is a modern, minimally invasive solution that targets the inflammation causing chronic heel pain. In many chronic cases, tiny abnormal blood vessels form around the plantar fascia—a process called neovascularization. These abnormal vessels contribute to ongoing inflammation and discomfort. PFE works by selectively blocking these abnormal vessels while preserving normal blood flow to the foot.

How the Procedure Works

  1. Local anesthesia & light sedation keep you comfortable throughout the procedure.
  2. A small puncture is made in the ankle or groin, where a microcatheter is inserted.
  3. Using real-time imaging, the physician guides the catheter to the arteries supplying the inflamed plantar fascia.
  4. Embolic particles are injected to block abnormal blood vessels and reduce inflammation.
  5. The catheter is removed, and a small bandage is applied—no stitches required.

Posterior Tibial Nerve Block & Peripheral Nerve Stimulation (PNS)

Chronic heel pain is often perpetuated by nerve irritation. Posterior Tibial Nerve therapies work by disrupting pain signals traveling from the foot to the brain.

Posterior Tibial Nerve Block

A nerve block uses a precise injection of anesthetic near the posterior tibial nerve at the ankle. This temporarily interrupts nerve signaling, providing immediate pain relief. Ultrasound guidance ensures accurate placement and avoids surrounding structures.

Peripheral Nerve Stimulation (PNS)

PNS is a longer-lasting neuromodulation therapy. A small lead is placed near the posterior tibial nerve to deliver gentle electrical pulses that modify pain signaling. This is ideal for patients with chronic, localized pain not relieved by other therapies.

Benefits of Minimally Invasive Treatments

For more severe PAD or when symptoms are not adequately managed by lifestyle changes and medications, a procedure may be necessary to restore blood flow:

Outpatient procedures with quick recovery

Rapid return to daily activities—often within 1–2 days

No large incisions—only a tiny needle puncture

Long-lasting relief by treating the underlying source of inflammation

Minimal discomfort and minimal scarring

Proven success rates, with many patients achieving significant pain reduction for years
